So, I really dont know where to start, and I kinda want to get organized before I have to go back to school Tuesday. Where do you start when you go back on your diet, change the kind of bread you eat, the milk you drink. Im so lost right now. Please help me. :)

I start with counting calories.....but make sure you are eating at least three meals a day.....four to five light ones if you can manage.
Try oatmeal, NOT smothered in sugar just a bit for taste, and fruit for breakfast...or if your only choice is cereal...make sure you take the recommended serving size.
lunch can be sandwiches...but make sure you are not sabotaging yourself with tons of may or cheeses....keep it simple...meat/bread/lettuce and veggies...maybe a little mustard. add some fruit to round it off and drink water.
dinners should be lean meats and veggies. simple. if you do pasta, again, measure out how much you are truly eating and go light on sauces..e.specially cream based like alfredo....limit yourself to one piece of bread it offered...and load up on veggies.
drink water.
lots of water.
it's hard to help you figure out where to start w/out knowing where you are. but you CAN do it! just be aware of what you are eating.
if you can, plan your meals out for the week *or a few days at least* and try to add in thirty minutes of exercise....you don't need a treadmill or a gym membership....do jumping jacks, run in place, sit ups/push ups, leg lifts. just MOVE for thirty minutes...dance if you need to.
